Serious incident

On Thursday, October 15, 2026, a worker died in an incident at Northern Crossarm, a treated wood products manufacturer in Chippewa Falls, Wisconsin, according to WEAU 13 News and ISSSource, which cited a U.S. Department of Labor spokesperson confirming that OSHA has an open investigation into the fatal incident.

No other details, including the worker’s name or what happened, have been released. OSHA has up to six months to complete its investigation.

When details are not public, the lesson is to look at your own plant: which jobs put people near moving equipment, loads, openings, or heights, and which defenses, such as guards, lockout, barriers, and fall protection, are in place for each one.
